Job Description:

We are seeking a dedicated and compassionate Physical Therapist to join our team at Home Therapy & Wellness. As a Physical Therapist, you will play a critical role in assessing, diagnosing, and treating patients with physical impairments resulting from injury, illness, or aging. Your goal will be to help patients restore mobility, strength, and overall function through customized physical therapy interventions, promoting independence and enhancing quality of life.

Key Responsibilities:

  • Conduct thorough evaluations of patients’ physical limitations, mobility, and strength to determine appropriate treatment strategies.
  • Develop and implement individualized treatment plans tailored to each patient’s specific needs and goals.
  • Provide hands-on therapeutic interventions, including exercises to improve strength, balance, flexibility, and range of motion.
  • Educate patients and their families on exercises, mobility techniques, and strategies to improve physical function and prevent further injury.
  • Collaborate with other healthcare professionals, including physicians, occupational therapists, and speech therapists, to ensure a comprehensive and holistic approach to patient care.
  • Maintain accurate records of treatment plans, progress notes, and therapy outcomes.
  • Participate in team meetings and case discussions to contribute insights and provide input on patient care strategies.
  • Stay informed about the latest physical therapy practices and incorporate evidence-based techniques into patient care.
  • Effectively manage a caseload to ensure timely and high-quality care for each patient.

Qualifications:

  • A degree in Physical Therapy from an accredited program.
  • Current state licensure or eligibility for state licensure as a Physical Therapist.
  • Minimum of 2 years of clinical experience, preferably in a rehabilitation or senior care setting.
  • Strong clinical assessment, reasoning, and problem-solving skills.
  • Excellent communication and interpersonal skills, with the ability to build rapport with patients and their families.
  • Compassionate and patient-centered approach to care.
  • Ability to work independently and collaboratively within a multidisciplinary team.
